Patient Qualifications
The Free Medical Clinic of the Ozarks is dedicated to providing quality health care to meet the physical needs of the uninsured of Stone and Taney County, MO. 

Eligible patients meet the following conditions:
  • Proof of residency within Stone or Taney County
  • Income qualifies
  • No health insurance

Make an Appointment
To make an appointment you must come in to the clinic during office hours to fill out a patient registration.  This is only required for your first appointment.  Meanwhile, please read a copy of the HIPAA Notice of Privacy Practices.

Additionally, you must bring in or fax a copy of the following:
  • Your driver's license or ID
  • A utility bill showing your address is within Stone or Taney county (not required if your ID shows residency within Stone or Taney county)
  • Pay check stub, recent taxes, or unemployment letter showing proof of income or that you are unemployed
